BACKGROUND
Humana is measured on patient satisfaction and value of care through the Centers for Medicare and Medicaid Services Stars Program. Humana's Stars Team was looking for new ways to create innovative and personalized experiences for their Group Medicare plan members to improve member health and satisfaction, preventative care and managing chronic conditions.
ASSIGNMENT
Create and track a multi-channel engagement web platform to facilitate sending customized communications to unlimited cohorts based on targeted data points, including disease states and annual gap compliance. Improve patient satisfaction and compliance, and by extension, Humana's Stars scores.
OUTCOMES
- Created requirements and roadmap, and managed the development and QA team to deliver results in an agile workflow.
- Led the UX and UI teams to create brand-approved visual designs and experiences.
- Managed data acquisition, including patient health and claims data from Humana, social determinants of health data, and motivation and lifestyle data collected through the platform. Created data schemas for all data coming in and out of the program.
- Built logic for cohort creation to enable personalized communications and managed content creation.
- Ensured compliance with Humana’s and CMS’s requirements for member communications.
- Established a design system within the Humana brand guidelines for template-driven programs via web, email and text that facilitated creating custom communications at scale.
- Managed the creation of an analytics dashboard to measure and visualize program performance.
